SERVICES

ASSIST TO INDEPENDENCE provides the following services to consumers:

  • Information and Referral – Staff provide information and referrals to consumers to connect them with programs and services that help them live independently.
     
  • Independent Living Skills Training -providing one-on-one and group training to assist consumers to live independently.
     
  • Transition Services – Assisting consumers with moving from nursing homes to less restrictive living arrangements, such as a home or apartment; from youth to adulthood; and from rehabilitation settings to the community.
     
  • Peer Support – Providing peer mentoring and support to consumers who are learning to live independently, from role models who have been there.
     
  • Individual & Systems Advocacy – Teaching individuals about self determination strategies and addressing physical and attitudinal barriers in society.

                Work Skills Education,
 

                Personal Assistance Services,
 

                Socialization and recreation,
 

                Sports, Fitness and Health,
 

                Home Modifications,
 

                Reintegration from nursing home to the community,
 

                Access to affordable health care and accessible housing,
 

                Transportation,
 

                Traditional Healing Services,
 

                Durable Medical Equipment.
 

WHO IS ELIGIBLE FOR SERVICES?
 

Assist To Independence uses various funds from different sources so eligibility criterias will vary depending upon funding source. Assist To Independence serves individuals who reside on the Navajo Reservation, Hopi Reservation, San Juan Southern Paiute Reservation, and individuals living in Navajo, Apache and Coconino  Counties.
 

 All applicants must meet the following eligibility criteria:

  • Must have physical, mental,      cognitive, or sensory impairment that substantially limits one or more of      the individual’s major life activity.
  • Must have a medical record of      such an impairment; or
  • Is regarded as having such an      impairment.
